Clinical Science Certificate Application Form
Students interested in applying for admission to the Clinical Psychological Science Certificate must have a 3.0 GPA or higher in their major and have completed Introductory Psychology and Statistical Techniques with a C+ or higher to apply (students may apply the semester they are enrolled in statistics). Students must complete this application and submit:
1-2 pages in length
2. Letter of intent describing your interest in the program, previous work experience (volunteer, paid employment, etc.), and career goals, which must include the relevance of the Certificate to those goals.
3. Academic Advising Report Copy of the your unofficial Academic Advising Report verifying current major GPA

- Interviews may be conducted with applicant finalists to determine admittance.
- Students will be selected based on a combination of GPA, demonstrated interest in the Certificate, having had appropriate prior work experience, and career goals which align with the priorities of the Certificate program. Minority and other underserved students are
strongly encouraged to apply.
