Formation of Advisory Committee for the Single Major
All students are required to form an advisory committee within their first year.

Formation of Advisory Committee for the Double Major
All students are required to form an advisory committee within their first year.
Instructions:

2. Download and complete (please type) the form Application to Change from a Single to a Double Major For the Ph.D. Degree. Obtain the required signature and return the completed forms to the Academic Services Coordinator in Psychology 126. Be certain to obtain the signature of the Chair or Director of Graduate Studies for the second major prior to submitting the form to the Academic Services Coordinator. You may also email the Academic Services Coordinator the form information and he/she will prepare a form for you. Please allow at least two days.Qualifying Examination Petition: To schedule your exam
Instructions: Complete the form and obtain committee signatures. Submit form to the Academic Services Coordinator in Psychology, Room 126.
There are two versions of the form.
Departmental Guidelines for the Qualifying Examination:
1. Qualifying examinations will be taken in the summer after the 4th semester in the program.
2. All written parts of the examination must be completed by the Friday before classes begin in the fall semester
3. If an oral examination is to be held, the entire process (including the examination and the evaluation of it) must be completed within four weeks following the completion of the written part of the examination (i.e., it must be completed by the Friday of the fourth week of classes in the fall semester). By that time, the faculty on the committee must be prepared to give a report to the full department faculty.
4. If the student does not pass the exam, all procedures used to let the student complete or re-take the exam must be complete by the end of the fall semester (i.e., the end of the fifth semester). The faculty on the committee must be prepared to give a report to the full department faculty by the end of the final examination week of the fall semester.
5. Students who are pursuing majors from two different departments or programs may be given one extra year to complete their psychology qualifying examinations.
6. Written applications for extensions on the examination must be made by the Friday of finals week of the fourth semester to the qualifying examination committee for approval or disapproval.

Qualifying Examination Results Form: To take your exam.
Instructions: Have committee complete and sign, indicating your results. Submit form to the Academic Services Coordinator.
Requirements for Admission to Candidacy: 39 credit hours (excluding research hours and P660). Typically P553, P554, 12 hours in major, 9 hours in minor (or have completed courses in 2nd major), 2 breadth, and 2 P595. You must also have passed the qualifying exam and submitted your First and Second Research Projects, completed P660 and taught one semester.
Instructions: Download the appropriate form. Complete only the name, ID, mailing address, and date of birth sections. Obtain your committee signatures and submit the form to the Academic Services Coordinator in Psychology, Room 126. The Academic Services Coordinator will complete the remainder of the form. DOUBLE MAJORS: Be certain to obtain the signature of your second major’s chair/director of graduate studies before submitting the form to the Academic Services Coordinator.

Nomination of Research Committee (i.e., Dissertation)
SINGLE AND DOUBLE MAJORS NEED A TOTAL OF AT LEAST 4 COMMITTEE MEMBERS.
This committee must be formed at least 6 months prior to the defense of your dissertation. All committee members must be members of the University Graduate School and at least half must be full members. You must include prospectus and relevant forms (e.g., IRB or animal use) with this form. SINGLE MAJORS Need two or more faculty from the department and one for each minor (at least 4 members required). DOUBLE MAJORS need a committee made up of at least four members, two from each major. A minor is not required for double majors. For both forms, please indicate committee chair.
Instructions: Download the appropriate form. Complete (type) only the fields for name, ID, date of birth, major/minor, and the title of your dissertation. Print the form and attach your prospectus and IRB forms. Obtain committee signatures and submit form to the Academic Services Coordinator in Psychology, Room 126. DOUBLE MAJORS: Obtain the signature of the chair/director of graduate studies of your second major prior to submitting the form to the Academic Services Coordinator. You may also email the Academic Services Coordinator the form information and he/she will prepare a form for you. Please allow at least two days.
Occasionally, students may need to change the members on his or her committees. Instructions: Download and complete (type) the appropriate form, print and obtain signatures. Submit the completed form to the Academic Services Coordinator. If you are a double major, be certain to obtain the signature of your second major’s chair or director of graduate studies prior to submitting the form to the Academic Services Coordinator. You may also email the Academic Services Coordinator the form information and he/she will prepare a form for you. Please allow at least two days.
